
The Colorado Attorney General's Office provides funding for the Colorado Opioid Model to Advance Treatment (CO-MAT) project through the Opioid Abatement Innovation Challenge. The goal of the CO-MAT project is to help Colorado counties and organizations expand opioid treatment and care coordination. At the heart of this project is the hub and spoke treatment model, an evidence-based approach that improves care coordination and services for individuals with Opioid Use Disorder (OUD).
Using the hub and spoke model, CO-MAT has three aims:
We are partnering with subject matter experts from Denver, Larimer, and Weld County, with the North Colorado Health Alliance as a primary partner. Our aim is to provide technical assistance to correctional care facilities to enhance Medication-Assisted Treatment (MAT) provision and post-release care coordination for people with OUD. Denver Health’s CAM Academy has released a free Opioid Use Disorder (OUD) Hub and Spoke Toolkit, designed to strengthen Colorado’s systems of care for individuals with OUD. The toolkit includes implementation guidance, adaptable templates, referral workflows, and real-world case studies to help communities expand treatment access and improve care coordination.
